Responsibilities
- Maintain confidentiality within the limits of safety to self and others.
- Help you understand any diagnosis, recommendations and treatment.
- Explain any test results when testing is finished.
- Develop a treatment plan that is suitable for you.
- Make a written statement of recommendations available to you.
- Make referrals for additional services as needed.
- Register at the Outpatient Registration desk for your initial appointment. If you are coming here for ongoing treatment, you must re-register for your first appointment in January and in your first appointment in July. If you have a change in name, address, phone number, insurance, etc., please let us know. In addition, you must stop at the Outpatient Registration desk to update their records.
- Complete forms and questionnaires as needed.
- Call if you cannot make your appointment. (24 hours advance notice is appreciated.) We reserve the right to discontinue services if there are an inordinate number of cancellations or no shows for your appointment. You will be notified by mail or by phone if this occurs.
- Notify us if you are seeing another therapist during the time of your assessment and/or treatment here, we legally need to know the name of the other provider and his/her role in your care. If this is an evaluation, it is expected that you will continue with your treatment during the course of the assessment.
- Check with your insurance company if you have any questions about your coverage. Some insurance carriers will not cover services to two different mental health providers at the same time. Charges for our services include treatment time, testing, meetings, verbal and written communications with persons that you specify and report time.
- Contact Crisis Services (1-800-724-0461) if an emergency occurs and your regular doctor or therapist can not be reached.
