Scheduling Your Visit
Oftentimes, your physician or your child's physician will call our office to make a referral. All referrals are physician based except those services provided by a facility contract. If you have not spoken to the physician, please contact his/her office and ask him/her to either mail or fax us a prescription.
After we have obtained the referral information, we will call you to schedule the initial appointment. If we are not able to reach you, you may call us at (716) 664-8563. If we are still unable to make contact, we will write a letter to you asking you to call to schedule the first appointment.
NOTE: If your insurance is Independent Health, your primary care physician must complete an insurance referral.
You must call to pre-register for our services. The phone number to call is (716) 664-8617. If you call and get voice mail, please leave the following information for the registrar so that he/she can return your call:
- your name
- the patient's name
- the phone number where you can be reached
- the best time for him/her to return your call
- and the date/time of your initial Psychology visit.
You will still need to stop at the Outpatient Registration desk when you come in for the first visit to sign papers consenting to treatment here and to give WCA permission to bill your insurance carrier.
You will not have to stop at the registration desk again except under the following circumstances:
- Insurance Change
- Name Change
- Address Change
- Phone Number Change
- To sign a waiver stating you will be financially responsible for charges that are uncovered services by your insurance company
- Your first appointment on or after January 1 of each year
- Your first appointment on or after July 1 of each year
We are located at the Jones Memorial Health Center. You come into the building that says in big letters, OUTPATIENT AND ADMISSIONS. You stop at the Outpatient Registration desk to sign and pick up paperwork. You will then take the elevator down to the first floor. When you get off the elevator, you will make a left hand turn and go all the way down to the end of the hallway and around the corner. You will come to a large waiting area. Please check in with the receptionist at the desk. Tell her the patient's name and the name of the person you are scheduled to see.
The number of appointments needed will vary depending on the individual circumstance.
If your first appointment is to complete the questionnaires, someone will come out to get you. While you are completing your questionnaires, a clinical staff member will stop in to introduce himself/herself and will answer any questions you may have about the assessment process.
The questionnaires for adults and children are an important way for us to identify your strengths and problem areas. When you are finished with the questionnaires, we will schedule the other appointments if necessary. The second appointment is usually an interview with the clinician.
